Job Description
McGregor PACE is a Program of All-inclusive Care for the Elderly (PACE site), the alternative to nursing home placement providing services that address the medical, rehabilitative, social and personal healthcare needs enabling seniors to remain at home, in the community.
We are seeking a Geriatric Nurse Practitioner to work in collaboration with our Primary Care Physicians and Interdisciplinary team in a supportive practice environment, with cohesive team members! The position requires an energetic applicant who enjoys working in a team setting designed to support elders at risk for nursing home placement, in addition to having a set panel of patients for care provision.
Hours are Monday through Friday, day shift only covering our Warrensville Hts and Lorain office locations
Essential Job Functions:
  • Works in a collegial role with primary care physicians in order to deliver comprehensive health services to their panel of participants, including: assessment, diagnosis, clinical decision making, treatment and prescribing, in the direct management of acute and chronic illness, and the promotion of wellness; follows participants into various care settings in collaboration with IDT.
  • Provides initial, semi-annual and periodic assessments for participants as requested
  • Multidisciplinary team support includes formal reassessments with reevaluation of the care plans that are to be completed on discharges from any acute skill admission as needed; assists in the coordination and documentation of a 24-hour care delivery for participants with input from all disciplines of the multidisciplinary team; on-going assessments of participants with a focus on physical, mental or emotional changes that are to be reported to the multidisciplinary team and reflected on the plan of care as needed
  • Identifies participant and family education needs and provides appropriate teaching/information
  • Works collaboratively with the Clinical Services Officer to identify learning needs of departmental personnel and makes recommendations for/participates in the presentation of continuing education; serves as a clinical resource for nursing staff and preceptor for clinical orientation
  • Assists in departmental and participant outcome data collection and analysis; participates in performance improvement initiatives, as relevant
  • Is responsible for identifying the scope of practice permitted by Ohio and Federal laws and regulations, the professional code of ethics, and professional practice standards
  • Responsible for achieving continuing education and other requirements for licensure, certification and prescriptive authority
  • Participates in clinical on-call rotation as requested
Qualifications:
  • Masters prepared nurse with certification as a Nurse Practitioner in Gerontology, holding active, Ohio Certificate of Authority, required. Prescriptive privileges, preferred
  • Minimum of five years direct care experience working with frail, older adults in community health, long term care, or hospital required
  • Minimum of two years advanced practice nursing experience, required
  • Previous experience in multidisciplinary care setting, desirable
  • CPR (BLS) certification, required
